Objectives:Orlistat (Xenical, Roche) was a novel, lipase inhibitor that has been approved for the management of obesity in Europe. The purpose of this article was to analyze the efficacy and safety of Xenical (Xenical, Roche) in obese patients who had not achieved weight loss. The primary outcomes of interest included BMI, total weight gain and BMI changes.

Methods:A retrospective cohort study was conducted in 1432 obese patients with and without a body mass index (BMI) ≥30 kg/m2 or with a body mass index (BMI) >30 kg/m2 or BMI ≥27 kg/m2. Patients were stratified according to the baseline BMI (BMI ≥30 kg/m2) using a modified BMI chart that included the following information: age, sex, ethnicity, race, BMI, ethnicity, socioeconomic status (including jobs), occupation and personal income. The primary outcome measure included weight gain, and the secondary outcome measure was BMI change from baseline to endpoint. The primary efficacy outcome was BMI change from baseline to endpoint.

A total of 547 patients were included. Of these, 295 patients with a BMI ≥30 kg/m2 (n=349), and 277 patients with a BMI ≥27 kg/m2 (n=265) were excluded. Of the 278 patients with a BMI <27 kg/m2 (n=398), 8 patients had a BMI ≥27 kg/m2 (n=209) and 9 patients had a BMI ≥27 kg/m2 (n=62). Overall, the median weight gain was 0.5 kg/m2 (95% CI, -0.6 to -0.2) at baseline and 0.2 kg/m2 (95% CI, -0.2 to 0.1) at follow-up in the Xenical group. Similarly, there were no significant differences between the Xenical and placebo groups in the change from baseline to endpoint. Patients in the Xenical group had a mean change from baseline of 0.0 kg/m2 (95% CI, -0.1 to 1.0) at baseline and 0.1 kg/m2 (95% CI, 0.1 to 0.3) at follow-up.
